Its actually there is old tradition in japan to dress a little boy who has weak body with girl clothes until they reach adulthood to fool evil spirit or something like that so they cant take the boy , i think nio design based on that except his clothes kinda questionable😅

So a few points, to just put them out there.

1. Yuen hates them all (maybe except Kurima, currently) and is using them and Kurima to further his goals. Which seem to be to dismantle the cult (which he has alluded to as being "a false prophet" in the previous episode. So while the rest of the revengers can be seen as outcasts with nowhere to go (except the Doc, who might be an avid follower of the cult), Yuen has an ideology and going to cleanse the cult that extra-judicially executes the people in the name of "god", staining it. Also, who knows where the money is going. That Hisarizawa-police guy, to me, feels like an actual ally for Yuen along the way. Especially since Nio had something bad to say about him.

2.Nio hates Yuen as well. When Yuen is asked if they can change, Yuen hesitates and, definitely, lies that they cannot. Otherwise it would seem like Yuen has been pushing them into this revenge-business while they could try leading proper, normal lives. At least this is the implication I got.

3. Given that this is Urobuchi Gen's work and the opening, we can expect every main character to perish by the finale. In the finale of the opening, Yuen is sort of leading them into a raging fire, and all 5 disappear in it. Could also be representative of the order they perish, but I would like to believe Yuen vs. Kurima is the finale.

After watching episodes 4 and 5, I think that your second bullet is incorrect: Yuen does not hate Nio. I am still not sure about bullet (1), and bullet (3) is probably true

It's not about hating Nio per se, but hating them all for what they are - "monsters" with no morals, who are ready to commit sins for fun or, for some, for religious reasons. He is using them to further his own ends and if he deems them unworthy of "salvation" - would cast them aside, but likely just orchestrate their untimely demise. This is how I see his character so far, based on clues, and it does not have to be true, maybe Urobuchi can surprise even me, or maybe I am understanding his stories quite well now.

I also don't think I fully grasp the scene with the addicted and Doc. Were they delirious and mistook the doctor for a government worker, who got them addicted in the first place? That seems like the implied reading, to me.

Bruh, there are drug addicts by choice and dense/vulnerable people who are pushed drugs.

In this episode they clearly point out that the samurai LEO was the one who pushed the drugs into the people. In a later scene, we see the girls ask for the "medicine" they were promised.

I would definitely call them sick in this scenario juxtaposed to drug addicts.
